Better Ideas
To help spread best practices, the National Initiative for Children’s Healthcare Quality (NICHQ) developed this implementation guide for the assessment, prevention, and treatment of childhood obesity. The practices are based on the June 2007 AMA/HRSA/CDC Expert Recommendation Guidelines. Additional tools and resources are also available on NICHQ's website as part of their Childhood Obesity Action Network.

Cancer Navigator includes two major components: a website and a call center. The website serves as a central repository of links to cancer information and services in San Diego County and includes links to national websites. The HelpLine provides personalized support, available in English and Spanish, for those who do not have Internet access or those who would rather speak with a staff member.

The National Guideline Clearinghouse (NGC), an initiative of the Agency for Healthcare Research and Quality (AHRQ), is a public resource for evidence-based clinical practice guidelines. The NGC site contains abstracts and full-text clinical practice guidelines, guideline comparisons, a searchable bibliography database for literature citations, and a discussion forum for exchanging ideas about guidelines.

NICHQ seeks to catalyze a national commitment to improve health care for children, as well as to achieve better outcomes in practices that care for children by developing, testing and deploying new methods to provide and improve care.

Improving Chronic Illness Care (ICIC), a national program of The Robert Wood Johnson Foundation, is dedicated to the idea that United States health care can do better. The 99 million Americans who suffer from diabetes, depression and other chronic conditions can lead healthier lives. Providers who care for chronically ill patients can be better supported with guidelines, specialty expertise and information systems. Overall health care costs can be lowered through better care delivery.

Leadership
The Health Resources and Services Administration (HRSA) Bureau of Primary Health Care (BPHC), in partnership with the Institute for Healthcare Improvement, has embarked on a nationwide initiative to improve care for people with chronic conditions. The Health Disparities Collaboratives strive to achieve excellence through goals such as building strategic partnerships, transforming clinical practice through models of care, improvement, and learning, developing infrastructure, and generating/documenting improved health outcomes. The website includes training manuals, tools and resources, and upcoming opportunities to get involved.

Self-Management / Adherence
This site documents a six-month pilot Collaborative, based on IHI's Breakthrough Series Collaborative model, which ended in April 2004. The Collaborative, funded by The Robert Wood Johnson Foundation and chaired by Doriane Miller of Health Research and Educational Trust (HRET), explored ambitious aims related to patient self-management of chronic conditions. Not all elements of the site are available to the public but the "Tools" section is especially rich.

The 2005 NHDR tracks disparities in both quality of and access to health care in the United States for both the general population and for designated priority populations. The report focuses on four components of quality—effectiveness, patient safety, timeliness, and patient-centeredness—and two components of access—facilitators and barriers to health care and health care utilization.

The CDC's National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) accomplishes its mission by providing national leadership in areas of health promotion and chronic disease prevention by conducting public health surveillance, epidemiologic studies, and behavioral interventions; by disseminating guidelines and recommendations; and by assisting state health and education agencies to increase their capacity to prevent chronic diseases and promote healthful behaviors.
