Improvement Report: An Integrated Approach to Improving Patient Care
At CareSouth Carolina (Hartsville, South Carolina, USA), a community health center serving the most medically underserved populations in our community, we have developed an integrated approach to improving patient care. We serve the rural South Carolina counties of Darlington, Chesterfield, Dillon, Marlborough, and Lee. By utilizing the Chronic Care and Open Access Models, we have significantly improved care for all of our patients, including patients with depression, asthma, diabetes, and cardiovascular disease.

Health Disparities Collaboratives: Improving Diabetes Care in 3,400 Health Center Sites
In 1998 the Bureau of Primary Health Care (BPHC), a division of the US Department of Health and Human Services, teamed up with the Institute for Healthcare Improvement to form the Health Disparities Collaboratives to improve diabetes care for patients. The BPHC oversees programs that offer care to twelve million people who are underserved by the United States health care system — the underinsured and the uninsured.

Member Report: Redesigning Diabetes Care Delivery System
Clinica Campesina Family Health Services (Lafayette and Northwest Adams County, Colorado, USA) redesigned its diabetes care delivery system for all Type I and II diabetic patients to improve patient self-management skills, glycemic and BP control; specific goals include average HbA1c <8 and 90 percent of diabetic patients will have 2 HbA1c’s >3 months apart every year.
