The ICU Insulin Order Set gives providers an effective way to provide consistent management of blood glucose levels while a patient is in the intensive care unit (ICU). The order set has three different options available for the Critical Care Physician to choose when the patient is initially admitted to the ICU. All three options are clearly presented and color coded. There are also directions for each option regarding information related to treatment, as well as specific notations as to when a particular option may be discontinued and another option started.
Directions
When a patient is admitted to the ICU, the Critical Care Physician places the patient on one of three options for blood glucose control, based on their history and present blood glucose level. Based on the option selected for the patient, as well as on the patient's actual blood glucose level, the nurse checks blood sugar levels at a particular interval of time and treats accordingly. “Triggers” help providers determine when a patient should be switched from one option to another, when more or less aggressive treatment is needed. Blood glucose levels are monitored and reported to the leadership team on a monthly basis. Based on that information and current research, changes have been made to the order set, though on an infrequent basis.
