General
Improving Chronic Illness Care (ICIC), a national program of The Robert Wood Johnson Foundation, is dedicated to the idea that United States health care can do better. The 99 million Americans who suffer from diabetes, depression and other chronic conditions can lead healthier lives. Providers who care for chronically ill patients can be better supported with guidelines, specialty expertise and information systems. Overall health care costs can be lowered through better care delivery.

HIV Guidelines and Education
AIDSinfo, supported by the US Department of Health and Human Services (DHHS), updates consumers and health care providers with current, federally-approved clinical research, treatment and prevention information, and clinical guidelines for HIV/AIDS care. The site also serves as a clearinghouse for federally and privately funded clinical trials for AIDS patients and others infected with HIV.

This website equips HIV health care providers with current, state-of-the-art tools to ensure delivery of the highest-quality HIV clinical care. The site includes HIV clinical guidelines, best practices related to HIV clinical care, quality improvement materials, and clinical performance data. International Resources
The World Health Organization, in conjunction with UNAIDS, created a working group in 1996 to strengthen global structures and networks for improved monitoring and surveillance of HIV/AIDS and STDs. The working group collaborates closely with national AIDS programs and a number of national and international institutions and experts. Their goal is to make the best information available to improve decision making about HIV/AIDS at national, regional, and global levels.

The National Health Care for the Homeless Council and the HCH Clinicians' Network involve homeless service providers and their clients in political action to reduce poverty and to prevent and end homelessness. Quality Improvement Resources
The National HIV Quality of Care (HIVQUAL) Project, sponsored by the HIV/AIDS Bureau of the Health Resources and Services Administration and carried out by the New York State Department of Health AIDS Institute, was established to improve the quality of care for persons living with HIV/AIDS by building capacity and capability for quality improvement in HIV ambulatory care clinics.

Through its four Bureaus (key program areas), HRSA works to improve and extend life for people living with HIV/AIDS, provide primary health care to medically underserved people, serve women and children through state programs, and train a health workforce that is both diverse and motivated to work in underserved communities.

The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) evaluates and accredits nearly 17,000 health care organizations and programs in the United States. An independent, not-for-profit organization, JCAHO is the nation's predominant standards-setting and accrediting body in health care. Since 1951, JCAHO has developed state-of-the-art, professionally based standards and evaluated the compliance of health care organizations against these benchmarks. Its mission is to continuously improve the safety and quality of care provided to the public through the provision of health care accreditation and related services that support performance improvement in health care organizations. In support of this mission, in 2002, JCAHO introduced Shared Visions-New Pathways, an initiative that focuses accreditation on operational systems that are critical to the safety and quality of patient care.

Self Management/Consumer Involvement
As a part of the Black AIDS Institute, the first Black HIV/AIDS policy center aimed at reducing health disparities in Black communities, the African American HIV University (AAHU) develops a team of highly-qualified and informed African American peer treatment educators through training, immersion, and technical support, and then places and supports them in their ongoing peer education efforts.

The AIDS Action Committee of Massachusetts (AAC) works to stop the HIV/AIDS pandemic by preventing new infections and optimizing the health of individuals already living with HIV and AIDS. The AAC offers support to those living with HIV, educates HIV health care providers, and informs the public about infection prevention. It also advocates for fair and effective AIDS policy on the city, state, and federal levels. Specific consumer resources hosted on the AAC website include discussions addressing how to talk to your doctor, where to get treatment and an interactive “ask a health question” feature.
