Adherence Readiness Assessment

Christiana Care Health Services
Wilmington, Delaware, USA

This tool is used to help a patient/client be more adherent to their HIV medications by identifying possible barriers and helping the client overcome them.

Directions

When it is determined that a patient/client needs to begin HIV medication based on their CD4 and viral load, the client and provider should complete this form in its entirety at the initial assessment visit. Upon completion of the form, the client and provider may also identify possible barriers to medication adherence. A team may then work to help reduce such barriers so that the client will adhere to all medications.
