The Institute for Healthcare Improvement's (IHI’s) 5 Million Lives Campaign targets a reduction of five million instances of harm from December 2006 through December 2008. Together with complementary partner initiatives, the Campaign is intended to act as a major driver of national improvement. This article, the first in a series, gives an overview of the predecessor 100,000 Lives Campaign, and describes the 5 Million Lives Campaign and its six new interventions.

This report features the first-ever comprehensive comparison of US health system performance. All 50 states are compared to benchmarks and ranked on 32 performance indicators of access, quality, avoidable hospital use and costs, equity, and "healthy lives." Some states ranked highly on multiple indicators, but none scored top marks in every area. The report concludes that federal and state action in key areas is necessary to achieve higher levels of performance and value.

In 2002 Ascension Health, a 67-hospital not-for-profit health care system, embarked on a journey of clinical transformation to eliminate preventable injuries or deaths. This transformational change implies a much greater pace of change than that reflected in traditional, incremental change processes. Their improvement activities focused on eight priorities for action: JCAHO National Patient Safety Goals; preventable mortality; adverse drug events; falls; pressure ulcers; surgical complications; nosocomial infections; and perinatal safety. Eliminating birth trauma at Ascension Health

Mazza F, Kitchens J, Kerr S, Markovich A, Best M, Sparkman LP. Eliminating birth trauma at Ascension Health. Joint Commission Journal on Quality and Patient Safety. 2007 Jan;33(1):15-24.

Ascension Health identified perinatal safety as one of eight priorities for action in a system-wide effort to achieve zero preventable injuries and deaths by July 2008. Three alpha sites developed and implemented transformational practices aimed at eliminating preventable birth trauma.
