Patient-Centered Care: General

The Institute for Healthcare Improvement and many other organizations have developed tools to help health care providers accelerate their efforts to provide more patient- and family-centered care and support. These tools include successful protocols, order sets and forms, instructions and guidelines for implementing key changes. We are making them available on IHI.org for others to use or adapt in their own needs. We also invite you to submit tools you have found useful!
