The purpose of this tool is to reduce variation in the administration of anticoagulants by creating one standardized physician order set. Previously, physicians used multiple order sets and this practice presented opportunities for using the wrong order set, thus leading to patient harm or an adverse event. The new protocol creates a single process for weight changes, ensures necessary lab tests are ordered, and aids in nurse/physician ease of use.
Background
This tool was developed in our efforts to reduce adverse drug events, particularly those related to high-risk medications, in conjunction with the Institute for Healthcare Improvement’s Reducing Adverse Drug Events Collaborative. The tool consolidates many different physician order sets into one standardized order set. This nomogram is the only Heparin order set to be used by all medical staff housewide.
