The IHI-led Pursuing Equity Action Community was designed to help participating health care organizations make progress on: clinical equity outcomes at the project level; process measures at the population level; scaling up a health equity intervention or demonstrating the ability to sustain improvements in equity; building capacity to measure and sustain improvements in health equity results; and building capability to close equity gaps.
This report:
- Describes the Pursuing Equity Action Community in detail, including its focus, goals, key learning, and challenges; and
- Summarizes the experiences and learning of the 10 participating health care organizations, including approach, progress and key successes, critical moments, challenges, and next steps.
The experiences of the participating organizations underscore the importance of data-driven approaches, leadership commitment, multidisciplinary collaboration, and community engagement to improve health equity. As health systems continue to strive for more equitable care, the lessons learned from this initiative serve as a valuable guide.
